Mailing Instructions
After making your purchase follow the steps below:
 
  1) Cut your hair and provide us a hair sample.

 Hair Sample - Wash & Cut Requirements

Put the hair sample in a small envelope or make one with printer paper and tape.

 2) Print, fill out the "Health Form" and sign it.
 3) Mail the Hair Sample and Health Form to:
The Body Balanced - Health Program
                   P.O. Box 1675
               Garner, NC 27529
Note: If paying by "check" add it with the hair sample and health form.
           Make Check Payable to:
